Print

Dear Staff and Students

The Health Economics and HIV and AIDS Research Division (HEARD), within the College of Law and Management Studies, is conducting a short survey to get your perspective on COVID-19 vaccines. We want to hear about your experiences with COVID-19 and how it has shaped your decision to vaccinate. For those who have chosen not to vaccinate, we want to hear your concerns and preferences for COVID-19 vaccine services.

Your participation is voluntary and confidential. Once we close the survey, we will be randomly drawing 10 participating staff or students who will win themselves a R 500 voucher. Your staff or student numbers will not be linked to your responses.

Attached is the consent form. Kindly note that you will be directed to the consent form again when you click to the survey (no need for signiture just tick to participate).

Approval letter to conduct this survey is attached.

Your participation is critical for improving our response to COVID-19.
